Diabetes Self Management Education

A Good Idea

Description

White River Rural Health's Diabetes Self Management Education provides a comprehensive assessment, individual education and classroom education with a Registered Nurse and a Registered Dietician for a comprehensive review of diabetes and related care. This is followed by a 3 month, 6 month and 12 month reassessment of the patient with the patient setting behavioral change goals and evaluation of those goals. With Diabetes Self-Management education patients have a better chance to avoid complications associated with uncontrolled diabetes.

The goals of this project are:
-to promote individual control of diabetes.
-to help patients become partners with their healthcare providers in the care of their disease.
-to help diabetes patients realize that small continous changes do make big differences.

Results / Accomplishments

There are now seven ADA Certified Diabetes Education Centers run by White River Rural Health Center. Classes are held 3 times a month. Participant response has been positive.
