Plan Implementation and Monitoring
With the Community Health Needs Assessment completed, the roles of the team members will expand to include communication, health issue progress assessment and documentation of implementation efforts. Each team member should be aware of communication responsibilities, which are critical to implementation of the plan, and will lay the groundwork for future community health needs assessments. New team members may need to be recruited to the team.
Determine how progress will be measured. What data should be collected to determine if progress is being made? Once data is collected, does it indicate movement in the right direction? Unsatisfactory progress should result in further analysis of what is not working and what may need to be adjusted. Modifications may range from minor adjustments to more robust changes in the plan.
