IKK and Community Health Needs Assessment (CHNA) Crosswalk
This crosswalk outlines the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) process in broad terms. The CHA/CHIP process assists cities, counties and states to utilize data to make informed decisions regarding prioritization of health issues and implementing health improvement efforts. The Kansas Health Matters website is designed to assist communities in linking data to best practices for health improvements.
The IKK coalition is a strong example of how this process can be put into action. The crosswalk shows how the IKK coalition used data, prioritized the issues affecting low immunization rates, and communicated with their stakeholders about the issues they identified. Then the coalition conducted a gap analysis, reviewed evidence-based interventions, and put together plans for action and monitoring efforts. Finally, the IKK coalition implemented the actcess along the way.
This is an important example to note because the CHA/CHIP process is meant to be iterative and flexible to fit individual community needs. The principles of the processes can be built into existing areas of work and provide a systematic way to approach areas that need improvement.
